This is the ED, or Emergency Department, at Penn State Milton S. Hershey Medical Center. Just outside, blue and white ambulances line up in neat rows, wheeling patients in through an oversized sliding door with “Ambulance Entrance” written atop in glowing, red letters. Some patients walk into the ED on their own, entering through another door. If a patient has no report coming in, they will get triaged, or screened, by nurses. Right now in triage, a woman sits in a wheelchair, speaking in hushed tones with a nurse, fielding questions about her medical history and concerns. But many patients who pass through this area don’t stop at triage: their situation is too dire, needs too imminent. Soon, trauma doctors and nurses will be fighting to save their lives. Each year, around 2,000 patients are admitted to the adult trauma service at Hershey Medical Center. Not only does the hospital offer trauma care, it is the only level one trauma center in the region for both pediatric and adult patients. This specialized subset of care means that doctors and nurses treat the sickest of the sick patients.

HARRISBURG — It's a picturesque Tuesday afternoon in early November and Marshall "Jess" Freer finds himself in a familiar spot. He steers the van from its parking spot on North 12th Street and turns right on Walnut Street. Another call comes across the radio — one of Freer's agents confirms the operation is a go. Freer hits the gas and takes a sharp left turn on North Summit Street. Within the blink of an eye, half a dozen police vehicles descend upon this tiny, one-way road and Freer's agents and Harrisburg police officers pour out into the street. After a quick call to a family member to arrange a ride for the children, Thomas is carted away in a Harrisburg police transport van. One could call this arrest another successful operation for Freer's squad — the state Attorney General's Mobile Street Crimes Unit. Created last year as state Attorney General Kathleen Kane amped up efforts to crack down on the state's growing heroin epidemic , the unit aims to partner with local law enforcement in areas where police resources are lacking.
